The papillomatosis of the bile duct may occur at any age but more often at the age of 40 and 50 with a slight preference for men. The main symptom is an icterus often combined with right-sided epigastric pain. ERC and PTC allows the preoperative diagnosis on the basis of the typical clinical picture. The papillomatosis of the bile duct has to be considered as a disease with cancer risk. The prognosis in case of incomplete removal is unfavorable because of the continuous growth, the intrahepatic spread and the elevated degenerative tendency. Therefore a complete removal should be considered.

Vitamin B12 deficiency was found in 10 of 41 patients who underwent radiotherapy before cystectomy with Bricker urinary diversion for carcinoma of the bladder. Of 13 patients given full irradiation because of inoperable bladder cancer 5 had malabsorption of vitamin B12. Serum folic acid was normal in these patients, indicating predominantly ileal irradiation sequelae. Routine evaluation of serum vitamin B12 after radiotherapy is recommended so that appropriate medication can be given, if possible before neurological symptoms appear.

A false-positive I-131 whole body scan (WBS) for lung uptake in a patient with papillary thyroid cancer, with replacement of his infiltrated esophagus with a left colon graft, is presented. The I-131 image, seen in the upper right lung field, was due to the contaminated colon graft lacking normal peristaltic movements, which was located in the right paramediastinal region. This finding was demonstrated by imaging the upper digestive tract with orally administered Tc-99m DTPA and by a barium x-ray.

The major host defense mechanisms against bacterial overgrowth in the small bowel are the normal propulsive activity of the bowel itself and gastric acid secretion. Microbial interactions are a major factor in regulating the indigenous bacterial flora. Studies of the bacterial enzymes of the gut suggest that changes in diet may lead to marked changes in the colonic flora. Antibiotics affect the composition of the colonic microflora. The microflora also influence the degradation of mucin, the conversion of urobilin to urobilinogen, of cholesterol to coprostanol, and the production of short chain fatty acids. Current interests are focused on the bacterial flora of tropical sprue, the role of bacteria in colorectal cancer, and the involvement of intestinal microflora in the enterohepatic circulation of sex steroid hormones.

The effect of in vitro hyperthermia on the immune response of murine and human lymphocytes was investigated. Viability, blastogenesis and cytotoxic activity of murine lymphocytes were reduced at 39 and 42 degrees C hyperthermia. In human lymphocytes, rosette formation inhibition depended on heating-time and temperature. PHA-induced lymphocyte blastogenesis was markedly inhibited at 42 degrees C, irrespective of the duration of exposure and of the origin of the cells, although it was enhanced in some patients at 39 degrees C. Therefore, the possibility of hyperthermia-induced inhibition of the host immune response must be considered in clinical trials of total-body hyperthermia for cancer.

The incidence, treatment regimens, and survival rates for nasopharyngeal carcinoma in China, one of the most common cancers in that country, are reviewed. Although treated almost exclusively with megavoltage photon therapy, in some cancer centers intracavitary radium insertions are used to increase the radiation dose to the primary tumor site. A histological study of these tumors shows that patients whose tumors are well-differentiated and contain an abundance of lymphocytes have the best prognosis.

Immunocytochemistry has been used to visualise the distribution of the anti-cancer drug VP16-213 in tissues from normal and leukaemic mice, and in EMT6 mammary tumours. Uptake of the drug into EMT6 tumour tissue was poor compared to uptake into normal tissues (liver, kidney, heart, small intestine). Uptake of the drug into tissues from L1210 mice was greater than that into normal tissues. Immunocytochemistry promises to be a very useful additional technique for studying the distribution of drugs given singly or in combination, specific organ toxicities, and mechanisms of resistance.

In this study, 26 patients were treated with 20 mg cisplatin (CP)/m2 body surface daily for 5 days as part of a combination therapy for testicular cancer. Simultaneously a vigorous hyperhydration (3000 ml daily) was applied to reduce the nephrotoxic effect induced by the drug, 131I-orthoiodohippurate clearance values obtained by the modified method of Oberhausen did not show any significant evidence of tubular dysfunction after therapy. These results may have been due to the vigorous hydration as well as to the youth of the patients.

This case report describes the unanticipated results of a transrectal ultrasound performed to stage a cancer of the rectum. During sonography, the normally dense prostate peripheral zone produced weak echoes indicating the existence of an unexpected carcinoma of the prostate. Primary hemochromatosis is a genetic disorder rarely recognized in childhood; its long-term consequences include cirrhosis and liver cancer. We report a family with primary hemochromatosis affecting three generations, including a 7-year-old child and a 29-month-old child; these are the youngest children with primary hemochromatosis yet reported. The pathophysiology, genetics, and clinical findings of this disorder are reviewed. Serum ferritin and transferrin saturation are useful screening tests; definitive diagnosis, however, depends on determination of hepatic iron content. A plan for evaluating and treating affected patients is proposed. Physicians caring for children must learn to recognize this potentially treatable disorder.

Thirteen patients with mandibular defects after radical resection of mandibular cancer from Apr. 1984 to May 1987 are reported. There were 9 males and 4 females. The defects were repaired by free vascular pedicle iliac bone grafts. Of these patients, 11 satisfactorily healed as the grafted bone was examined by ECT scan autoradiography. The latter is considered as a reliable objective monitor and a scientific parameter to evaluate the viability of the grafted bone. Wide surgical indication, improved post-operative function, shortened treatment course in one-stage repair are favorable to the patients.

TITLE: Operative strategy for thyroid cancer. Is total thyroidectomy worth the price?

The authors conducted a retrospective analysis with 5- to 30-year follow-up on 109 patients in order to determine the optimum management of nonmedullary thyroid cancer. Results of total thyroidectomy were compared to partial thyroidectomy, among patients well matched for prognostic indicators. No differences in cancer mortality or recurrence rates were evident. However, there were significantly more complications when total thyroidectomy was employed. In view of these results, partial thyroidectomy is recommended as the treatment of choice for nonmedullary thyroid cancer.
