TITLE: Hypercoagulopathy induced by chemotherapy in a patient with lung cancer. A possible role for a factor with thrombosis-inducing activity (TIA).

We treated a patient with lung cancer in whom a hypercoagulopathy was induced acutely by chemotherapy. He received systemic chemotherapy twice and in both instances developed disseminated intravascular coagulopathy (DIC), accompanied by acute decrements of the peripheral platelet count and plasma fibrinogen, an increment of the fibrin degradation products (FDP), and bleeding tendency with the appearance of skin purpura. In each instance, the plasma thrombosis-inducing activity (TIA) appeared one to three days after chemotherapy and subsided subsequently.

TITLE: Ectopic atrial natriuretic peptide production in small cell lung cancer with the syndrome of inappropriate antidiuretic hormone secretion.

One autopsy case with small cell lung cancer and the syndrome of inappropriate antidiuretic hormone secretion (SIADH) is reported. Both plasma atrial natriuretic peptide (ANP) and arginine vasopressin (AVP) levels were high, and the presence of significantly high levels of ANP and AVP in tumor tissue was confirmed by gel chromatography and radioimmunoassay techniques. To the best of the authors' knowledge, this is the first case in which ectopic ANP production and its secretion into blood (leading to SIADH) were proved.

At the moment, proton-therapy is the most advanced radiotherapeutic technique in cancer treatment. The use of the high energy proton beam (from 70 MeV to 200 MeV) lets a Bragg's peak be moved to different depths, so allowing personal radiotherapeutic treatment. In recent years, many proton-therapy centers have grown up throughout the world with very satisfactory clinical results, first of all in eye melanoma treatment. The future expectations are very promising, even if the very high installation and maintenance expenses of a synchrotron (for proton production) hinder the development of such a method.

Free radial forearm flap was used to replace the tissue defect caused by pull-through operation for cancer of the mouth in 13 cases. There was no total or partial flap failure. The greatest advantage of the method is that the restriction of the tongue movement is less when compared to the results of other methods since the thin forearm flaps are well adaptable and are of a pliable nature. The newly developed lower alveolar crest makes the prosthetic rehabilitation possible.

The inhaled tobacco smoke is the most important determinant in the etiology of lung cancer and there is a clear dose-effect relation between lung cancer risk and the duration of smoking, the number of cigarettes and the intensity of inhalation. The opinions about lung cancer risk from passive smoking are divided and the published data are still in discussion. Occupational substances include chromates, nickel, beryllium, alkylated compounds, vinylchloride, arsenic compounds and in particular asbestos. Fortunately air pollution has no or small part in the etiology of lung cancer.

Detection of HBxAg was done by the immunogold-silver staining technique in 22 liver cancer specimens from patients with hepatocellular carcinoma and positive markers of hepatitis B were found to express HBxAg of hepatitis B virus, which was distributed in the perinuclear cytoplasm. The result suggests that HBxAg can be detected practically in all the liver specimens of the patients with hepatocellular carcinoma, who had been infected by hepatitis B virus. The relationship between X gene and its translation product and hepatocellular carcinoma is discussed.

The frontline chemotherapy for advanced breast cancer has been either CMF or CAF and the combinations can obtain an overall response rate exceeding 50%, including complete responses of 10-15% and median survival times of 12-18 months. Complete responders have survived longer than patients with partial responses or stable diseases. Consequently, high dose chemotherapy with autologous bone marrow transplantation has been investigated in order to obtain more complete responses and ultimately to obtain a cure in patients responding to induction chemotherapy. Seventeen to 30% of patients who became complete responses have maintained continuously complete responders. Taxol and CI941 are promising agents under investigation.

Bowenoid papulosis, erythroplasia of Queyrat, and Bowen's disease are histologically identical to in situ carcinoma. Although the first is consistently benign, the latter two regularly evolve into invasive cancer. Malignant scrotal lesions include squamous-cell carcinoma, liposarcoma, leiomyosarcoma, basal-cell carcinoma, extramammary Paget's disease, erythroplasia of Queyrat, malignant melanoma, and metastases. Hemangioma can be confused with carcinoma.

Amplification of mdr-1 occurs in breast cancer patients receiving endocrine treatment. We have compared a group of patients receiving tamoxifen and a group receiving second-line endocrine treatment after failure of tamoxifen treatment. Chronological tumor samples were obtained by fine-needle biopsies from the same tumor lesion of each patient and PCR was used to detect the appearance of gene amplification. Three of 20 patients receiving tamoxifen developed amplified mdr-1 whereas 7 of 17 patients receiving second-line endocrine treatment developed mdr-1 amplification.
