Regional chemotherapy is an interesting treatment option in patients with advanced pancreatic cancer but cannot be considered standard treatment, and it should not be performed outside controlled clinical trials. The real value of regional chemotherapy must be evaluated in larger, randomized trials. New drug combinations may reduce the observed side effects and improve tumor response. Gene therapy with p53 and K-ras modulated herpesviruses may become a palliative treatment option and can be administered easily by regional chemotherapy techniques [23].

Hence, apoptosis may be relevant as therapeutic targets for many human diseases. This article reviews briefly the regulation and the clinical relevance of apoptotic mechanisms in several different human diseases.

By the early 1980s, epidemiologists had identified many important causes of cancer. They had also proposed the 'multi-stage' model of cancer, although none of the hypothesized events in human carcinogenesis had then been identified. The remarkable advances in cell and molecular biology over the past two decades have transformed the scope and methods of cancer epidemiology. There have been a few new discoveries based purely on traditional methods, and many long-suspected minor risks have been estimated more precisely. A simple method for testing the assumption of independent censoring is developed using a Cox proportional hazards regression model with a time-dependent covariate. This method involves further follow-up of a subset of lost-to-follow-up censored subjects. An adjusted estimator of the survivor function is obtained for the dependent censoring model under a proportional hazards alternative. The proposed procedure is applied to an example of a clinical trial for lung cancer and a simulation study is given for investigating the power of the proposed test.

Patients with lung cancer often develop airway obstruction that necessitates palliative bronchoscopic interventions. Expandable metal stents are increasingly used for this indication. We report a case of fatal massive hemoptysis that occurred 1 week after uneventful bronchial stent placement. Previous treatment with external-beam radiotherapy, brachytherapy, and photodynamic therapy probably contributed to pulmonary artery-bronchial fistulization in this patient. This complication should be kept in mind when considering expandable airway stent insertion in heavily pretreated lung cancer patients.

Here, a novel approach to genome expression pattern interpretation is described and applied to the recognition of B-cell malignancies as a test set. Using cDNA microarrays data generated by a previous study, a neural network model known as simplified fuzzy ARTMAP is able to identify normal and diffuse large B-cell lymphoma (DLBCL) patients. Furthermore, it discovers the distinction between patients with molecularly distinct forms of DLBCL without previous knowledge of those subtypes.

In developed societies, chronic diseases such as diabetes, obesity, atherosclerosis and cancer are responsible for most deaths. These ailments have complex causes involving genetic, environmental and nutritional factors. There is evidence that a group of closely related nuclear receptors, called peroxisome proliferator-activated receptors (PPARs), may be involved in these diseases. This, together with the fact that PPAR activity can be modulated by drugs such as thiazolidinediones and fibrates, has instigated a huge research effort into PPARs. Here we present the latest developments in the PPAR field, with particular emphasis on the physiological function of PPARs during various nutritional states, and the possible role of PPARs in several chronic diseases.
