Newly diagnosed cancer patients who were screened as being "at risk" for future psychosocial distress were offered a counseling program, covering the period from hospital discharge to resumption of regular activity. More than two-thirds of "at risk" patients accepted. Those who refused the program tended to be more antagonistic or apprehensive than those who accepted. Refusers denied difficulties, minimized problems, and in certain instances were truculent and suspicious. Some refusers did not wish to be interviewed, for fear that social and emotional equilibrium would be disturbed; they rejected counseling, which was felt to be either an immediate threat or an omen of disaster. Accepters, while as much "at risk," did not deny, but felt more hopeless. Differences in cancer site, stage, symptoms, treatment, availability of significant others, age, sex, social status, and so forth were not significant.

The group of gastric and colorectal carcinomas has the highest cancer mortality in Western Germany. The possibilities of treatment with chemotherapy only have palliative character. 5-Fluourouracil is the therapeutic of choice on the metastasized colorectal cancer. However, a prolongation of survival time by treatment of the colorectal tumor could neither be recorded in applying 5-Fluorouracil as a single agent, nor through combination chemotherapy. The treatment of the metastasized gastric carcinoma with combinations consisting of the 5-Fluorouracil and Adriamycin could be of future value. Also effective with this tumour as a palliative measure is the combined treatment with 5-Fluorouracil and irradiation. The advantage of an adjuvant therapy of a chemotherapeutical, radiological or immunological kind could so far not be proved.

TITLE: Immunoglobulin class of agglutinins to oral microorganisms in post-irradiation cancer patients.

Fractionation of serum samples from head and neck cancer patients and their assay for agglutination activity toward oral microorganisms showed that the activity was derived from IgG, 7S IgA, 10S (dimeric) IgA, and IgM, with considerable activity in the 10S IgA region. These findings probably account for the lack of a significant relationship among levels of individual serum immunoglobulins, agglutination titers, and caries activity presented in an earlier study. Conversely, most of the agglutinating activity in fractionated saliva was attributed to 11S IgA, presumably secretory IgA, with some activity from a 19S substance and, in the caries-active patients, from a higher molecular weight factor. These data correspond to positive correlations between saliva IgA levels, agglutination titers, and absence of caries in these patients.

TITLE: Familial ovarian carcinoma: a problem in genetic counselling.

Ovarian papillary adenocarcinoma is reported in two families. In one family, this tumor was detected in four females, of whom two had had mammary cancer below the development of the ovarian tumor. In the second family, a mother and daughter died of ovarian adenocarcinoma. The mode of inheritance in both families fits the pattern of autosomal dominant transmission of a single gene. Both are Jewish families from Germany, presently residing in Israel. The two families are presented as examples illustrating the difficult question of genetic counselling in families with recurrent cases of ovarian carcinoma. The difficulty arises because this tumor is relatively frequent but is usually observed in isolated cases without systematic follow-up in the patients' families. Only a few cases, where more than one person in a family is affected, have been selectively reported in the literature.

TITLE: Metastatic carcinoma of the pancreas simulating primary bronchogenic carcinoma.

A retrospective study of 294 patients with biopsy- or autopsy-proven adenocarcinoma of the pancreas was done. The initial diagnosis of ten patients (3.4%) was primary lung cancer. All ten patients were cigarette smokers. Hilar adenopathy with mediastinal widening was the most common roentgenographic appearance. Tumors of the body and tail of the pancreas more often appeared to be primary lung tumors than did tumors of the head of the pancreas (about 10% vs. about 1%). Adverse consequences of this unrecognized phenomenon may include unnecessary lung surgery for some cancer patients and overreporting of deaths from lung cancer.

Generally the cancer of the rectum is one besides several other diseases of the patient; clinically it recedes often far into the background of the actual happening. With increasing age the real geriatric diseases come to the foreground of the clinical findings dominated by the arteriosclerosis in all its variations. Therefore the treatment has to take into account the true secondary diseases besides the cancer. Judging the progress of the illness and the therapeutic effect these facts must be considered. Due to the intanglement of geriatric and oncological symptoms it is impossible to separate geriatrics from oncology. Concerning these connections it seems to be justified to characterize the rectum cancer--and also some other cancers--as a particularly geriatric one. The practical consequence of this hypothesis is a change of therapeutic planning, coordinating oncological and geriatric points of view according to the respective requirements. At this psychological and social-medical problems have to be taken into consideration.

TITLE: Protective effect of plant sterols against chemically induced colon tumors in rats.

Diets rich in vegetables are associated with a low incidence of colon cancer. Since plant sterols are plentiful in vegetarian diets, we studied the effect of beta-sitosterol on colon tumor formation in rats treated with the carcinogen N-methyl-N-nitrosourea. We demonstrated that beta-sitosterol nullified in part the effect of this direct-acting carcinogen on the colon. We suggest that plant sterols may have a protective dietary action action to retard colon tumor formation. The beneficial effects of vegetarian diets may be enhanced because of the presence of these compounds.

Estrogens and progesterone seem to have at times antagonistic, at times synergetic action on breast and uterine tissues. While estrogens stimulate cell proliferation, progestrerone favors secretion. While estrogens favor myometrial and endometrial hyperplasia, progesterone seems to dominish congestive phenomena caused by estrogens, especially on breast tissues. Women who experience menopause at 55 have a twice as high risk of breast cancer than women who menopause at 45. Breast and endometrial cancer are often associated with a history of menstrual disorders, and it is very possible that endometrial hyperplasia and cystic mastosis are connected. While treatment with estrogens helps to avoid vaginal atrophy and to arrest osteoporosis, it is doubtful whether the treatment should be systematic over 45. Breast examination and a thorough gynecological examination are essential once a year.

The tissues from 30 cases of endometrial cancer and 44 cases of cervical cancer were examined for oestrogen receptor activity. Twenty of the endometrial and 9 of the cervical tumours contained oestrogen receptor levels above 4 fmol/mg protein. The proportion of oestrogen receptor-positive tumours was significantly greater in adenocarcinomas of the cervix than in squamous carcinomas of the cervix. Tissues from 3 mixed mesodermal tumours of the uterus, 2 carcinomas of the vagina, a carcinoma in situ of the cervix and a carcinoma in situ of the endometrium were receptor-negative. One ovarian carcinoma and a single case of uterine sarcoma were receptor-positive. The implications of these findings in relation to hormonal therapies are discussed.

TITLE: [Changes in the bacterial flora of patients under antineoplastic treatment]

The bacterial flora of the skin from different anatomical sites on cancer patients and a control group of medical personnel was examined. This was done to ascertain if antineoplastic therapy was able to change the pattern of microbial flora of patients and to provide a control for possible infectious complications. The results show that in the control group bacterial flora was normal and the antineoplastic treatment did not succeed in changing the bacterial pattern in the skin of patients deeply. Gram negative bacteria were isolated more frequently from the skin of leukemia patients than from either patients with malignant melanoma or other neoplastic diseases.

Patients with advanced head and neck cancer have significant reduction in their circulating lymphocyte mass which is reflected in decreased numbers of T cells, Fc receptor cells, and in derangements of T lymphocyte functions, i.e., decreased responsiveness to several dilutions of phytohemagglutinin and lack of development of delayed hypersensitivity to 2,4-dinitrochlorobenzene (70% of patients were DNCB (-)). A population of phagocytic cells capable of decreasing lymphocyte responsiveness to mitogens were demonstrated. Removal of these cells resulted in an increment in lymphocyte responsiveness. Immunological impairment seemed to correlate with patients' ability to become sensitized to DNCB. Those patients who were DNCB (+) had less derangement in their immunological parameters. Weekly administration of Corynebacterium parvum in conjunction with chemotherapy did not have a discernible effect on patient's immune reactivity.

TITLE: Assessment of testicular function by the radioimmunoassay of testosterone in saliva.

A sensitive RIA (500 fg/tube) has been developed for assay of testosterone in male saliva and extensively validated. In normal male saliva samples, morning concentrations (368 +/- 167 pmol/l) were significantly higher than evening samples (212 +/- 132 pmol/l). The circadian rhythm was confirmed by COSINOR analysis. Levels of testosterone in saliva, in response to HCG stimulation, accurately reflected the increase observed in matched plasma samples. Synacthen administration, although increasing circulating cortisol levels, caused no significant change in plasma and salivary testosterone concentrations. Prostatic cancer patients on diethylstilboestrol therapy had low salivary (47--122 pmol/l) and plasma (1.0--2.8 nmol/l) testosterone concentrations. Correct assessment of testicular function following stimulation and treatment regimens requires multiple sampling. Since saliva samples are easily collected by non-invasive techniques they represent an attractive alternative to plasma for evaluation of androgenicity.

It has recently been claimed that degradation of folic acid to pterin-6-aldehyde (or 6-hydroxymethylpterin) is a property of tumour cells, but not of normal cells in culture, and that pterin-6-aldehyde is found in the urine of cancer patients only. The existence of such a cancer-specific metabolic pathway, if confirmed, would have tremendous significance for all branches of cancer biology and medicine. The results presented in this paper, however, demonstrate that normal human skin fibroblasts in culture readily degrade folic acid to pterin-6-aldehyde or a closely-related substance. Evidence is presented that this is a metabolic property of the normal cells themselves, and is not related to latent mycoplasma contamination.

TITLE: Tolerance of surgery after radical radiotherapy of carcinoma of the oropharynx.

The complications of salvage surgery after radical irradiation were studied in a group of 52 patients with oropharyngeal carcinomas. These patients were selected for surgery from a group of patients who had failed protocol therapy and who were registered in the Split-Course Study of the Radiation Therapy Oncology Group. The operative mortality was 1.9% (1/52). The most frequent serious complication was pharyngocutaneous fistua in 12% (6/52) of patients who underwent operations. The overall incidence of serious complications was 21% (11/52). The actuarial estimated five-year survival for the group who had undergone operation was 27%. We conclude that salvage surgery for residual or recurrent disease in oropharyngeal cancer after radical radiotherapy is feasible with a low surgical mortality and acceptable complication rates.

TITLE: Inhibition of estrogen synthesis in human breast tumors by testololactone and bromoandrostenedione.

The inhibition of aromatase enzyme in human breast tumors by delta 1-testololactone, testololactone, 6 alpha-bromoandrostenedione, and 6 beta-bromoandrostenedione was investigated. Estrone and estradiol synthesis from androstenedione was reduced in 3 tumor incubations by the presence of 0.13 mM delta 1-testololactone and testololactone. 6 alpha- and 6 beta-bromoandrostenedione (2.0 microM) were also shown to block estrogen synthesis in 2 tumors. Furthermore, Lineweaver-Burk plots revealed that all 4 compounds are competitive inhibitors of androstenedione aromatization. An apparent Km of the aromatase enzyme for androstenedione of 0.08 microM and a Vmax of 23 pmol of estrone synthesized/g tumor/hr were determined for one human breast tumor specimen. These results demonstrate that these aromatase inhibitors may be useful for the treatment of breast cancer.
