TITLE: Varying degrees of amplification of the N-ras oncogene in the human breast cancer cell line MCF-7.
PUBDATE: 19850501
The oncogene N-ras has been found to be amplified (congruent to 20 copies) in the human breast carcinoma cell line MCF-7. The amplified sequences have been localized to a marker chromosome by in situ hybridization. Sublines of MCF-7, serially passaged in different laboratories, have marked variation in the degree of N-ras amplification. The differing degrees of amplification of N-ras are further evidence of heterogeneity within MCF-7 subclones. The phenomenon may not have general relevance for breast cancer, since other breast cancer cell lines and DNA from patient biopsies failed to show evidence of N-ras amplification.

PUBDATE: 19851101
Progestin therapy of ovarian carcinoma in the past has been reported to lead to varying response rates. A multicenter phase II study of high-dose MPA was conducted in 53 patients with epithelial ovarian cancer who had received adequate trials of conventional therapy with cytotoxic agents. Forty-one patients were included for response and toxicity evaluations. Only one partial response has been recorded with a duration of 20 weeks. Stabilization of disease was observed in 7 patients. The present investigation shows that MPA given at the present high dose is not effective in patients extensively pretreated with chemotherapy.

TITLE: Prognostic factors in patients with non-small cell bronchogenic carcinoma and brain metastases.
PUBDATE: 19820501
Prognostic factors were examined in 38 patients with nonsmall cell lung carcinoma and brain metastases. The most important factors were the response to total therapy (corticosteroids, radiotherapy, and chemotherapy) and the presence of brain metastases alone; these factors had the most impact on survival. Age, sex, histologic type of lung cancer, and initial performance status were not prognostically important. Our results indicate that certain subgroups of patients with nonsmall cell lung carcinoma and brain metastases have a favorable prognosis and should be treated aggressively.

A nationwide study of all 3183 female patients with breast cancer in Israel diagnosed over a 7-year period (1960 to 1966) was conducted. Monthly series analysis showed a clear seasonal pattern in the symptomatology of the disease most pronounced in patients younger than 55 years, in all ethnic groups, mainly confined to cases with a nonlocalized tumor at diagnosis. Peaks occurred during spring, and troughs appeared during autumn. It is suggested that the pattern is of endogenous, probably hormonal nature, although environmental factors cannot be excluded.

PUBDATE: 19840601
Pala, 5G /M2 i.v. every 3 weeks was given to 32 evaluable patients with ovarian carcinoma (31 previously treated). No complete or partial responses were noted. Ten patients had stable disease for a median progression-free interval of 4.7+ months, while the remainder had progression of disease from the start of therapy. The major toxicity was dermatologic, and five patients had severe skin rash with desquamation or ulceration. There was no substantial hematologic or gastrointestinal toxicity. PALA displays no useful activity in previously-treated patients with ovarian cancer.

The clinical details of a case of primary carcinoma of the vagina are described. The literature on the management of vaginal cancer is reviewed. Treatment must be individualized and planned according to the location and stage of the primary tumour. All patients treated for malignant lesions of the vagina must be followed up for life with regular pelvic and vaginal cytological examinations.

Groups of patients with newly diagnosed metastatic breast cancer who were consecutively registered into a breast cancer clinic beginning January 1956 (210 patients), January 1966 (210 patients), and January 1975 (251 patients), were analyzed to determine the impact on survival of the various treatment modalities utilized in each decade. Comparisons of survival were made among subgroups of patients with similar prognostic features. Except for patients with very poor performance status, all subgroups showed a significantly improved survival in the 1970s compared with the earlier decades. Combination chemotherapy was considered to be the major factor in the survival improvement.

The results of a study on 30 patients with ear, nose and throat cancers are discussed; a comparison of clinical, ultrasound and histological findings for the preoperatory evaluation of the cervical lymph nodes has been carried out. Ultrasound technique is an excellent method of investigation for the staging "N" of such neoplasms; its contribution is very important for the following therapeutic program.

PUBDATE: 19820601
Standardized mortality rates for eight major types of cancer (as well as for all types of cancer considered together) in four South African population groups are compared with each other and with rates in selected countries. Wide differences among the races in South Africa are displayed, and several rates are shown to parallel those in other population groups abroad. This lends support to the belief that socio-economic factors (rather than solely genetic factors) play the major role in cancer causation.

The natural history and pathways by which carcinomas of the gallbladder and extrahepatic bile ducts spread after attempted curative surgery offer a rationale for postoperative radiation therapy. We review: 1) the increasing use of primary-curative radiation therapy for these cancers, along with 2) the early results of innovative intraoperative irradiation and the transluminal insertion of radioactive sources into transhepatic catheters, 3) the efficacy of irradiation for metastases to the portahepatis, and 4) the overall effects of irradiation on the gallbladder and extrahepatic biliary system.

Meningeal carcinomatosis due to primary ovarian carcinoma is an uncommon metastasis that presents difficult problems in diagnosis and management of patients known to have this disease. There is evidence that extended survival due to advances in treatment of ovarian carcinoma has improved our ability to diagnose meningeal carcinomatosis of this tumor more often. Herein the first case of meningeal metastasis of primary ovarian cancer with cytologic and postmortem examination is discussed.

PUBDATE: 19821201
Tumourous changes of the mamilla and the areola of the breast are rare compared to tumours of the glands of the breast. Although these tumours are rare a large number of possible tumours occur in this area. The differential diagnosis is therefore difficult. A survey of the possible tumours is given in tables and macroscopic pictures. The macroscopic similarity of the adenoma of the mamilla with primary cancer of the mamilla and pagets disease of the mamilla are stressed. The possibility of tumour like swellings which are not neoplastic is described. Among these pseudotumours locally infected conditions are predominant. Any tumour in the mamilla or areola which cannot be diagnosed with certainty must be subjected to microscopic tissue diagnosis.

PUBDATE: 19840401
In addition to the chromosome changes common to cancers, recent development of cytogenetics in human cancers has revealed, the site of specific chromosome changes which characterize specific cancers. Such cancer-specific aberrations are generally reciprocal translocations or deletions, and in some cases the translocations have been unequivocally demonstrated to involve cellular oncogenes (c-onc) in the new transcriptional control of other genes. Chromosome changes relatively common to cancers are amplification of specific genes by chromosome duplication or selective amplification as manifested by double minutes or homogeneously staining regions, and genome reorganization by nondisjunction, somatic recombination, polyploidization-segregation or other parasexual recombinations. The importance of such genome reorganization in cancer development was discussed in relation to the possible involvement of cellular escape mechanisms.

Preservation of the intercostobrachial nerve during axillary dissection in 50 patients with stage I breast cancer resulted in a significant improvement in sensory function compared with the standard dissection in which the nerve is routinely sacrificed. No local recurrence was found during the 3 to 5 year follow-up period. This modification is worthwhile in patients with axillas that are clinically negative for cancer.
