TITLE: Synthesis and biological properties of a 1-(6-aziridinylhexyl)-2-phenylindole, a potential fluorescence label for estrogen receptors.

2-Phenylindole derivative 8, linked to aziridine by a hexamethylene spacer group, was synthesized and tested. It binds to the estrogen receptor with an RBA-value of 4.0 (estradiol = 100). In vitro, it shows a selective cytostatic activity in hormone-dependent MCF-7 breast cancer cells (41% T/C at 10(-6) M). In vivo, it exerts a strong estrogenic effect. The binding to the estrogen receptor is largely reversible.

From 1980 to 1988 more than 1,549 organ donors were harvested, 125 of them (8%) died from cancer--116 brain tumors and 9 donors with extracerebral cancer. In 3 donors suffering from extracerebral cancer 5 kidneys were transplanted. All transplants functioned well without tumor signs up to 21 months after grafting. However, because of tumor recurrence in nearly 50% of the cases described in the literature generally organs from donors suffering from extracerebral cancer should not transplanted if possible.

The roles of history, physical examination, blood tests, such as thyroid function test, thyroglobulin (htg), calcitonin and CEA, radionuclide and ultrasound imaging, and fine-needle aspirates are outlined for the diagnosis and management of thyroid cancer. An algorithmic diagram of the monitoring procedures for long-term management of various forms of thyroid cancer is presented.

Despite doubts about the benefits of colon cancer screening, sigmoidoscopy and fecal occult blood tests are now often performed by clinicians who must then deal with positive test results. The purpose of this literature review is to examine potential strategies for dealing with positive screening test results. If positive results are obtained, current data support the use of colonoscopy as the single most effective diagnostic strategy. Exceptions include situations that reduce the predictive value of the screening test. Several important areas of controversy are highlighted that need to be assessed in subsequent studies.

We evaluated the relation between forced expiratory volume in 1 second (FEV1) and lung cancer incidence and mortality among members of the Kaiser Permanente Medical Care Program who took a multiphasic health checkup. FEV1 was inversely related to risk of lung cancer among former and current smokers, but not related among never-smokers. We observed a decreased risk of lung cancer mortality only in the higher quintiles of FEV1 in current smokers among men, but not in women. FEV1 appears to be associated with lung cancer as a physiologic marker for smoking-induced pulmonary damage.

Of the 140,000 cases of colorectal cancer anticipated this year, approximately 2% to 6% will occur in patients less than 40 years old. Data regarding colon cancer in this age group applies directly to the similarly aged active duty population. The problem is illustrated by two case histories. A review of the literature indicates that patients of this age group may have a worse prognosis, not due to age per se, but likely due to a combination of an advanced stage at diagnosis and a higher percentage of poorly differentiated or mucin-producing tumors. Early diagnosis remains essential for improved survival.

The immune response against murine IgG2a immunoglobulin was studied in 42 gastrointestinal cancer patients treated with mouse monoclonal antibodies. IgM, IgA, and all 4 IgG subclasses directed against mouse immunoglobulin were studied. Thirty of 42 patients developed an anti-mouse response of IgG isotype, often associated with IgM and IgA specific for mouse monoclonal antibodies circulating in serum. Patients first developed an IgM response, followed by IgA and IgG. IgG1 was found to be the most frequently expressed isotype, followed by IgG2 and IgG3. IgG4 was detected in only 6 patients.

A class of adaptive weighted log-rank statistics is described where the vector of weights is chosen in a data-dependent way from a family of "smooth" weight vectors. A parametric family of weight vectors is identified which includes most shapes of weighting vectors that will be near optimal in many cancer prevention and screening trials. This family of weight vectors is used in an application of the proposed method to data from a breast cancer screening trial. Results from a small simulation study comparing the power of the adaptive statistic to that of the unweighted log-rank statistic are presented.

A study comparing stomach cancer incidence rates between Puerto Rico and Puerto Rican migrants to New York City provided the foundation for this investigation.

Palliative therapy for rectal carcinoma includes control of bleeding, tenesmus, mucous discharge, obstruction and pain. These local symptoms can cause serious morbidity for patients with locally advanced or metastatic rectal cancer. As an alternative to surgical resection for palliation, the authors applied endoscopic laser fulguration in 15 patients with rectal tumours; 14 experienced immediate relief of symptoms. At 6.4 months' follow-up, two patients had recurrent tenesmus and two required a colostomy. A high degree of patient acceptance makes laser fulguration for rectal tumours an important modality, for palliation; however, control of tenesmus may be temporary.

The occurrence of metachronous colo-rectal cancer was investigated in 903 patients under the age of 40 years at the time of diagnosis of the primary cancer. A constant frequency of metachronous colo-rectal cancers was found and a cumulated incidence of 30% after an observation period of 41 years. Metachronous cancer of the colon was encountered most frequently in the right half of the colon regardless of the site of the primary cancer. On the basis of the above mentioned results, a life-long follow-up programme is recommended, consisting of regular examination for blood in the faeces and colonoscopy.

Clustering is normally used to group items that are similar. In this application of obtaining a diverse sample from the 230,000 compounds in the National Cancer Institute Repository, we cluster to select compounds that are different from the rest, to optimize screening for new leads. With these constraints, our approach yielded many singleton clusters. We can interpret these results as evidence for a limit to classification, contrary to the customary view of chemistry as a study of classes of compounds.
