This short historical review of the highlights of the history of pain management gives particular emphasis to the 20th century and to chronic and cancer pain.

Randomized trials and overviews on the value of altered fractionation for head and neck cancers are reviewed. Attention is drawn to the unexpectedly high incidence of temporal lobe necrosis incurred in patients with nasopharyngeal carcinoma. Preliminary analyses suggest that incomplete repair is likely to be the major factor, even a 6-h interfraction interval may be inadequate if substantial volumes of nervous tissues are included within the target volume.

There are currently no recommended screening tests for the early detection of ovarian cancer. Ovarian malignancies are most often discovered on bimanual exam at advanced stages, resulting in a high mortality from this disease. While the Papanicolaou (Pap) smear is not a reliable screen for the detection of ovarian cancer, abnormal cytology may be the initial indicator of ovarian malignancy. This is a case report of an invasive serous adenocarcinoma of the ovary that was discovered as the result of a clinical work-up for abnormal cytology reported on a Pap smear.

TITLE: Surgical management of solitary pulmonary nodules. An unexpected lucky case.

The case of a 63-year-old woman with a solitary pulmonary nodule (SPN) is reported. Surgical wedge resection revealed an hamartoma. Digital examination of the lung parenchyma showed an unexpected (not previously visualized by thoracic imaging) second nodule that intraoperative frozen examination revealed an adenocarcinoma. The operation was completed with a right upper lobectomy and complete lymphoadenectomy. The authors recommend an early open surgical procedure for any SPNs with risk factors for developing lung cancer.

The increasing occurrence of incidentally discovered benign adrenocortical tumors has become a clinical dilemma because of the difficulties in differentiating them from their malignant counterpart. Adrenocortical tumors are associated with familial cancer syndromes such as the Beckwith-Wiedemann syndrome, the Li-Fraumeni syndrome, the Carney complex, multiple endocrine neoplasia type 1, congenital adrenal hyperplasia, and the McCune-Albright syndrome. Genetic events are known to take place on the chromosomal and gene level in sporadic adrenocortical tumors.

TITLE: Herpes zoster in children with cancer.

The aim of this study was to determine the incidence and outcome of herpes zoster hospitalised children with cancer in Kota Baru. It was a retrospective review from January 1994 to December 1998. The diagnosis of herpes zoster was a clinical one. Herpes zoster was diagnosed in 10 of 188 (5%) children with malignancy. The most common malignancy was leukaemia. Nine children were treated with acyclovir. No child developed visceral dissemination and there were no deaths.

The application of molecular analysis of gene expression to clinical tissue samples represents one of the most exciting new areas in "translational" thyroid cancer research. Current data suggest that molecular diagnostic assays may improve the sensitivity and accuracy of fine needle aspiration of thyroid nodules, fine needle aspiration of metastases, and detection of recurrent disease in peripheral blood samples. It is likely that at least some of these tests will become useful adjuncts in the diagnostic armamentarium of clinical endocrinologists in the future.

With the introduction of BCG, intravesical instillation of immunotherapeutic agents has become a mainstay of therapy in the treatment of superficial bladder cancer. Interferon is capable of inducing a non-specific cellular and humoral immune response towards tumor cells. It has shown promise in reducing the recurrence and progression rates of superficial bladder cancer. In contrast to BCG, intravesical interferon is associated with minimal side effects and a very low dropout rate. Current research has focused on the use of interferon in combination with immunotherapeutic and cytotoxic drugs.

There was summarized the experience of treatment of the thyroid gland cancer, invading trachea (larynx) and other neighboring structures of the neck in 42 patients, whose state was estimated as severe one and neoplastic process--as nonoperable. Optimal surgical methods were applied depending on clinical signs of thyroidal carcinoma. In 36 patients the palliative resection of thyroid gland (in 6--with tracheostomy addition) was done, in 6--intervention was restricted by tracheostomy formation. The method of tracheostomy formation in cases of massive spreading of tumor along trachea is adduced.

The oesophagus is divided into four regions: cervical oesophagus, and intrathoracic oesophagus with an upper, mid- and lower thoracic portion. Cancer may occur on each of these regions. Computed tomography of the thorax and superior abdomen and endoscopic ultrasound are necessary for reliable staging. CT simulation allows accurate definition of tumour volume. GTV includes tumour volume and regional lymph nodes. CTV encompasses GTV plus a safety margin and lymph node areas considered to harbour potential microscopic disease. The extent of prophylactic lymph node irradiation depends on the anatomic location of the primary tumour.
